The free self-tissue transfer will be described here with breast reconstruction by abdomen tissue as an example.
This abdomen tissue is transferred on the place of removed breast and here connected with local vessels.
Here the so-called DIEP-Flap is concerned. DIEP means "deep inferior epigastric artery pedicled perforator".
The DIEP - Flap consists of skin and deep fat tissue, which is removed from the place, where it is excessive or even
often disturbing, namely in the abdomen area. Thereby during the careful preparation the vessels feeding tissue flap
are separated muscle-sparing from stomach muscles. Thus enough voluminous tissue with similar to the natural breast
consistence is obtained without any noticeable weakening of the abdomen wall stability.
The reducing of the excessive
abdomen tissue often represents once more additional esthetical profit. The advantage of this microsurgical self-tissue
transfer lies in the reducing of xenogenic materials (e.g. silicon implants) that require the follow-on operations during
the lifetime (e.g. implants replacement).
Free tissue transfer requires special educated and micro surgical experienced operation team as well as the equipping
of operation theatre with operation microscope for vessel recanalization. In similar way the self-tissue from buttocks
area or thigh can be used for the tissue transfer, especially when small breast volume is desired.
